Risk Management

The Ohio South Risk Management (now called "Kidsafe") e-system allows administrators, coaches and volunteers to complete an a Risk Management form and receive a certificate indicating you have supplied the necessary information to be in compliance with the OSYSA KidSafe program. This certificate can then be provided to your leagues, clubs, referee instructors, etc., who collect the forms indicating you have submitted the data required by the program.

Attention Referees: For Member League Code, look for the following choices in the drop down menu in the Risk Management Clearance Form (Rather than selecting your Member League):

District 1 Referee
District 2 Referee
District 3 Referee
District 4 Referee
District 5 Referee

Option 1:
Complete a Kidsafe/Risk Management Clearance Form...

Make sure you print enough copies of this certificate to provide to each of your leagues, referee associations or other entities that may require proof of your submission of data.

Option 2:
Fingerprint Background Check

Print the Kidsafe Form and Return to the OSYSA Office
