Have you used the Ohio South Online Tournament System before? If so, you may log in below by entering your Email address and street number, then select the form you need. If the system finds a match based upon the info you enter and form needed, the form will automatically populate.

Important Ohio South e-Travel Notes: Tournaments in US Youth Soccer have always relied on out of state teams providing a copy of their approved roster with the travel permit. With the use of the e-travel forms, rosters are no longer required for initial approval of the travel permit. Teams traveling to US Youth Soccer Tournaments are still required to provide a copy of their approved travel permit, approved league roster and their guest player roster to the tournament director. You may not be able to participate in a tournament unless you have all of these forms. OSYSA teams do not need permission to travel to an Ohio South sanctioned tournament.
