ODP Forms

Here are the forms that all state pool players must fill out for regional camp.  We understand that some players will be alternates but we collect from everyone so that we have it in case the player is called up to go to camp.

• Parent/Guardian Consent and Player Medical Release Form (PDF) 1 original
• Please note that insurance card also needs to accompany form above so here is an additional copy of the insurance card form
• ODP Medical Release Form (PDF) 1 original
• ODP Medical History Form (PDF) 1 original
• ODP Player Profile Form (PDF) 1 original
• ODP Player Code of Conduct Agreement (PDF)  1 original

Parents, please also keep a copy of each form above for your records.  This will make it easier if something happens to the original. On the Medical History Questionnaire, "Up to Date" vaccinations are not acceptable.  The actual dates must be entered.

ODP Players Who Cannot Attend ODP Practice:
