ODP Forms

 Here are the forms that all state pool players must fill out for regional camp.  We understand that some players will be alternates but we collect from everyone so that we have it in case the player is called up to go to camp.

• ODP Medical Release Form (PDF) 3 originals all notarized
• ODP Medical History Form (PDF) 1 original and 2 copies
• ODP Player Profile Form (PDF)
• Girls need 1 original and 2 copies with pictures attached to all three
• Boys need 1 original and 1 copy with pictures attached to both
• Proof of Player’s Medical Insurance Coverage (PDF) 3 copies
• ODP Player Code of Conduct Agreement (PDF) 
• Girls need 1 original and 2 copies
• Boys need 1 original and 1 copy

Tips: Please be sure that the number of originals and copies specified are turned in.  Double check to see that all forms requiring signatures are signed by the parent and player.  All Medical release forms must be notarized. Copies of notarized forms will not be accepted. Do not forget to attach pictures to the player profiles.  On the Medical History Questionnaire, "Up to Date" vaccinations are not acceptable.  The actual dates must be entered.

ODP Players Who Cannot Attend ODP Practice:
Click Here to notify ODP Administrators and the Ohio South Director of Coaching that you will not be able to attend an ODP Practice.
